    Program: Discount #2: Healthy Lifestyle Activity Points
    Description: To be eligible for Discount #2, you need to earn at least 50 points by participating in a variety of wellness programs, preventive exams/screenings, and/or wellness events throughout the year.

    To earn points toward this discount, please see the menu of wellness program options below available for you to participate in throughout the year.

    There are 8 wellness categories you can participate in. Each category focuses on a specific aspect of wellness. You may pick and choose from any of the categories to complete your Discount #2 points requirements. See tabs below for more details.

    There are three levels of points in which you can earn your Discount #2. Each category is worth $5 off your bi-weekly medical premiums for a total value of up to $15 per pay period.
    • Level 1 - 50 points: $5 total per pay period
    • Level 2 - 100 points: $10 total per period
    • Level 3 - 150+ points: $15 total per pay period
    Who can participate? All employees covered under the Pima County medical plan.
    Reporting Periods: March 1st - February 28th each year.
    Distribution Period: July 1st - June 30th each year
    How to view progress:

    Your records will be updated in the Employee Self-Service (ESS) Portal (ADP) by the 15th of the month following the date of verification of completion.

    100 Pushup Challenge: 20 Points

    100 pushups may seem like a lot - it can sound a bit daunting. If you're serious about increasing your strength, follow this 6 to 8 week training program and you'll soon be on your way to completing 100 consecutive pushups!

    200 Lunge Challenge: 20 Points

    If you're interested in building strength in your quadriceps and glutes, follow this 6 - 8 week training program and you should soon be on your way to completing 200 consecutive lunges! All you need is a solid plan, plenty of discipline and about 30 minutes a week to achieve this goal!

    Seasonal Fitness Challenge: 10 - 60 Points

    The Seasonal Fitness Challenge is easy and energizing! The goal of the challenge is simple: to accumulate at least 150 minutes of physical activity each week. 150 minutes is equal to an average of 20 minutes of exercise a day. Physical activity includes anything that requires moderate to active participation and is continuous for at least ten minutes. Each day you are physically active, you will track your total minutes on the Exercise Tracking Logs.

    Practice Prevention - Preventive Health Exams and Screenings

    You may participate in one or more of the listed preventive exams, services or programs to earn up to 50 Healthy Lifestyle Activity Points for this category only.

    Eligible health services include office visits to your physician, Primary Care Physician (PCP), nurse practitioner or other healthcare professional for routine physical exams. A routine exam is a medical exam given by a healthcare provider for a reason other than to diagnose or treat a suspected or identified illness or injury.

    Preventive health exams or screening services are covered at 100%. They include:

    • Annual Physical Exam
    • Routine Preventive Lab Panel
    • Mammography Preventive Exam
    • Prostate Preventive Exam
    • Colorectal Preventive Exam
    • Well Adult/Woman Exam

    NEW Preventive Exams/Screenings for FY2019/20:

    • Dental Preventive Exam or Cleaning
    • Flu Shot
    • Age Appropriate Routine Immunization
    • Vision Preventive Exam
    • Skin Cancer Screening

    Pima County Employee Wellness will receive an eligibility file from Aetna, Davis Vision and Ameritas each month which will include a list of employees who completed an eligible preventive exam or screening. 

    Note: Claims may take up to three months to process before being sent to Pima County Employee Wellness. Cap of 50 points for the Preventive Exams/Screenings category.

    Preventive Exam-Screening Affidavit

    Annual Physical Exam: 50 Points

    Typically includes medical history, blood pressure, a physical exam and routine lab work. A routine annual physical preventive exam is a medical exam given for a reason other than to diagnose or treat a suspected or identified illness or injury.

    Beginning Right Maternity Program: 50 Points

    With Aetna’s Beginning Right® Maternity Program, you can get special attention for a healthier pregnancy and baby. This no-cost program can give your baby a healthier start.

    Beginning Right Maternity Program helps you:

    • Make good decisions for your baby
    • Lower your risk for early labor
    • Stop smoking for good
    • Get special attention for pregnancy risks
    • Cope with postpartum depression and more

    Colorectal Preventive Exam: 50 Points

    Typically includes one of the following screenings for members age 45 and older and for adults under 45 who are determined to be higher risk. A routine colorectal cancer preventive screening is a medical screening given for a reason other than to diagnose or treat a suspected or identified illness or injury. 
    • Flexible Sigmoidoscopy every 5 years. Uses a flexible scope to evaluate changes and abnormalities in the lower part of the large intestine.
    • Colonoscopy every 10 years. Uses a long, flexible scope to detect changes or abnormalities in the large intestine (colon) and rectum.
    • Fecal Tests that screen for colon cancer including:
      • Immunochemical fecal occult blood test (iFOBT) annually. Small stool sample collected on a card or in a tube is sent to a lab to test for occult (hidden) blood that can indicate cancer.
      • Fecal Immunochemical Test (FIT) every 3 years. Small stool sample collected on a card or in a tube is sent to a lab to test for hidden blood that can indicate cancer.
      • Cologuard every 3 years. Available by prescription only. Small stool sample is mailed to a lab to be screened for blood and DNA cell changes linked to cancer.

    Dental Preventive Exam or Cleaning: 25 Points

    Regular dental exams are an important part of preventive health care. During a dental exam, the dentist or hygienist will clean your teeth and check for cavities and gum disease. The dentist or hygienist will also evaluate your risk of developing other oral health problems, as well as check your face, neck and mouth for abnormalities. A dental exam might also include dental X-rays (radiographs) or other diagnostic procedures.

    Flu Shot: 25 Points

    The flu is a contagious illness caused by influenza viruses that infect the lungs, throat and nose. The flu affects millions of people each year and can lead to serious illness, or even death. According to the Centers for Disease Control and Prevention (CDC), the best way to prevent the flu is by getting vaccinated each year.

    Earn 25 Healthy Lifestyle Activity Points for completing the program requirements.

    Age Appropriate Routine Immunization: 25 Points

    Eligible health services include immunizations for infectious diseases recommended by the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention. Doses, recommended ages, and recommended populations vary.
    • Diphtheria, pertussis, tetanus (DPT)
    • Hepatitis A and B
    • Human papillomavirus (HPV) for females and males aged 9 – 26 years
    • Influenza
    • Measles, mumps, rubella (MMR)
    • Meningococcal (meningitis)
    • Pneumococcal (pneumonia) for adults aged 65 and older
    • Shingles zoster vaccines, both Zostovax and Shringrix for adults age 50 and older.
    • Varicella (chicken pox)
    Earn 25 Healthy Lifestyle Activity Points for getting a preventive immunization.

    Mammography Preventive Exam: 50 Points

    Mammography provides an x-ray picture of the breast to check for breast cancer in women who have no signs or symptoms of the disease. Suggested every 1-2 years over age of 40 and for women under 40 determined to be at high risk. Includes 3D mammography. A routine mammography is a medical screening given for a reason other than to diagnose or treat a suspected or identified illness or injury.

    The M.O.M. van is scheduled at various County and City worksites during the months of February and August each year. Check back periodically to see the schedule of on-site locations.

    Prostate Preventive Exam: 50 Points

    Typically includes physical examination of the prostate and a prostate-specific antigen (PSA) screening. Considered preventive for men age 40 and older and for men under 40 years who are determined to be high risk for prostate cancer, including African American men and men with family history. A routine prostate preventive exam is a medical exam given for a reason other than to diagnose or treat a suspected or identified illness or injury.

    Routine Preventive Lab Panel: 50 Points

    Typically includes complete blood count (CBC) and a lipid profile to check cholesterol and triglyceride levels. Providers may decline ordering lab work or may request additional lab work depending upon your age and risk factors. You may be responsible for the costs of additional lab work.

    Skin Cancer Screening: 50 Points

    A visual inspection of the skin by a medical professional for purpose of detecting precancerous and cancerous skin growths. A routine skin cancer preventive screening is a medical screening given for a reason other than to diagnose or treat a suspected or identified illness or injury.

    Vision Preventive Exam: 25 Points

    Periodic eye and vision examinations are an important part of preventive health care. Many eye and vision problems have no obvious signs or symptoms, so you might not know a problem exists. Early diagnosis and treatment of eye and vision problems can help prevent vision loss.

    Earn 25 Healthy Lifestyle Activity Points for getting a preventive vision exam.

    Well Adult/Women Exam: 50 Points

    This exam includes an office visit to your physician, PCP, obstetrician (OB), gynecologist (GYN), OB/GYN or nurse practitioner. A routine well woman preventive exam is a medical exam given for a reason other than to diagnose or treat a suspected or identified illness or injury.

    The 5-week program will inspire you to focus on five key pillars of nutrition outlined in the Eat Right For Life book that will transform unhealthy behaviors into healthy habits that will last you a lifetime. You will receive a fun and inspiring copy of the Eat Right For Life book - FREE! that will help you adopt healthy eating habits in five key areas outlined in the book.

    Eat Smart: Fruit & Veggie Challenge: 15 - 25 Points

    Join this fun and motivating 4-week challenge that will help you to EAT SMART by offering simple suggestions to increasing your daily consumption of fruits and vegetables. The Log Sheet will make it easy for you to track your daily servings of fruits and vegetables. You earn 1 point for each serving you track, which can add up quickly.

    CupanionThis 2-month challenge is designed to encourage you to boost your daily consumption of water at home and work. You will receive a special tag to place on your favorite reusable water bottle to scan with the Fill it Forward app each time you refill. During the Fill It Up Hydration Challenge the goal is to fill up your reusable water bottle multiple times per day

    Health Assessment: 50 Points

    The health assessment is an important step in understanding the big picture of your health. It should take you about 15 minutes to complete.

    Health Coaching: 15 Points

    Meet one-on-one with a Health Coach from the Wellness Council of Arizona (WELCOAZ) for strategies and assistance in creating a plan to manage a health concern, whether you are looking to lose weight, exercise more, eat healthier, manage stress or lower blood pressure and cholesterol. All appointments are 30 minutes.

    Offered in December each year, the 12 Days of Wellness Challenge is designed to be a helpful tool you can use to keep yourself on track with your health. For most people, it can be a difficult time of year to keep one's health priority due to the many holiday activities going on, abundance of food and lack of physical activities. Complete a different wellness activity for each of the 12 days and record them on the 12 Days of Wellness Calendar.

    Aetna In Touch Care Program: 15 Points

    The In Touch Care program offers support and guidance for covered individuals and their family members looking to manage costly, chronic conditions, including asthma, diabetes, congestive heart failure, coronary heart disease, end-stage renal disease, depression, hypertension, arthritis, and more. Individuals with multiple conditions can also benefit from the In Touch Care program.

    Better Sleep Challenge: 20 Points

    When it comes to sleep, bad habits may be preventing you from getting the rest you need. The good news is bad habits are just that, habits, and they can be changed. The Better Sleep Challenge aims to inspire participants to create better sleep habits.

    Community Volunteering & Participation: 5 - 25 Points

    Show pride in your work and your community by volunteering your time and energy to a helpful cause. You are also encouraged to participate community events. Each 1 hour (up to 4 hours) dedicated to volunteering in an event will be worth 5 points. You may also participate in the event for an additional 5 points. Blood Donations are 5 points.

    Healthy Living Program (CDSMP): 50 Points

    Join this six-week interactive program developed by Stanford University and facilitated by the Pima County Health Department to help participants learn to better manage a chronic condition like diabetes, hypertension, heart & lung disease, chronic pain or mental health conditions, and more.

    Stress Less Programs

    Stress Less - Blood Pressure Challenge: 20 Points

    This challenge will help you learn stress-reduction techniques you can use to help lower your blood pressure throughout the day.

    Over the course of a month, you are asked to track your blood pressure before and after performing a stress-reduction activity on ten separate days using the health kiosk located at your work-site. If you do not have a health kiosk, please contact Employee Wellness at 724-2792 to borrow a blood pressure machine to share at your worksite.

    No Weight Gain Contest: 20 - 30 Points

    Most Americans gain an average of 1-3 pounds during the holidays – don’t be one of them! This program can help you stay focused on eating healthy and fitting in exercise during the holiday season so you can keep off those extra pounds. Your goal is to maintain your weight during the holiday season so that no more than 2 pounds is gained. Weight loss is encouraged, but not required.
